(Students who did NOT attend Elm Middle School as a 7^th grader, John Mills, Elmwood  or the Early Childhood Center last school year. Elmwood Park Residents only.)
 

1. PRINT and fill out the forms below.
2. CALL the Elm Middle Office at 708-452-3550 to make an appointment. The office will be closed from June 13, 2011 through July 22, 2011.

·         Illinois Department of Public Health Proof of School Dental Examination Form (Grades 2 & 6 only)

·         District Health Information Questionnaire

·         Illinois Student Transfer Form (please obtain from previous school—not required if coming from a private school)

·         District New Student Questionnaire

·         Elm School Request to Release School Records

·         Elm School 2011-2012 Payment Schedule
