DISCOUNTS OFFERED FOR EARLY REGISTRATION
Save up to $43.00 per student if you mail/drop off your payment and forms by July 30, 2010. See the Payment Schedule for discounted rates and deadlines.
 
 
First Day of School -Tuesday, August 17th
7:55am to 11:30 am
Bus service for Tuesday, August 17, 2010: Students should be at their bus stop at 7:20 am.
Buses will depart school at 11:40 am.

RETURNING STUDENTS
(Students who passed 6th grade at John Mills, Elmwood School and/or
Students who passed 7th grade at Elm Middle School for the 2009-2010 school year)
 
Students were given an EPCUSD401 Returning Student Proof of Residency Form and an Elm Middle School Fee Payment Form in early May to bring home.
 
MAIL your proofs of residency, residency form and payment to:

OR, DROP OFF your proofs of residency, residency form, fee payment and payment form in the High School Counseling Office anytime after May 10, 2010.
 
If you did not receive or misplaced your forms: you are able to print new forms from the attached links.

 
*FEE WAIVER
If you are applying for a fee waiver please come to open registration on August 4, 2010 or August 5, 2010 from 2:00 p.m. to 7:00 p.m. (Remember to bring documentation such as tax returns, unemployment pay stubs, etc.)
 
PAYMENT PLAN
Cheryl McCarthy, District Payroll Clerk, will be happy to set up a payment plan (promissory note) for you. You may contact her anytime after May 10, 2010. District Offices are open all summer Monday through Thursday, 8:00 a.m. to 3:00 p.m. Her direct line is 708-583-5842.
 
NO COMPUTER?
If you do not have access to a computer, please visit the Elmwood Park Public Library, located at One Conti Parkway (708-453-7645).  The library has computers for your use for a small fee. You may visit the High School Counseling Office over the summer to complete forms and pay fees in person. The High School Counseling Office is open all summer Monday through Thursday 8:00 a.m. to 3:00 p.m. 
 

NEW STUDENTS:
 (Students who did NOT attend John Mills, Elmwood or Elm Middle School last school year. Elmwood Park residents only.)
 

  1. PRINT and fill out the forms below.
  2. CALL the Elm Middle School Office at 708-452-3550 to make an appointment. The office will be closed from June 11, 2010 to July 25, 2010.
