(Students who did NOT attend Elmwood School or the Early Childhood Center last school year. Elmwood Park Residents only.) 

  1. PRINT and fill out the forms below.
  2. CALL the Elmwood School Office at 708-452-3558 to make an appointment. The office will be closed from June 13, 2011 to July 22, 2011.

·         Illinois Student Transfer Form (please obtain from previous school—not required if coming from a private school)
