Returning Students (Students who attended Elmwood School or the Early Childhood Center last school year.)
New Students (Students who did NOT attend Elmwood School or the Early Childhood Center last school year. Elmwood Park residents only.)

Wednesday, August 18, 2010 and every Wednesday during the school year:
9:17 a.m. to 3:00 p.m.
Thursday, August 19, 2010 and every Mon., Tues., Thurs., Fri.:
8:40 a.m. to 3:00 p.m.

The 2010-2011 Proof of Residency Form and Payment Schedule were sent home with students in May. If you misplaced those forms or if you didn’t receive them, please:
1. PRINT AND FILL OUT the forms below:
- EPCUSD401 Proof of Residency Form Returning Students (one per family)
- Elmwood 2010-2011 Payment Schedule* (one per student)
2. MAIL your proofs of residency and payment to:

If you are applying for a fee waiver please come to open registration on August 4, 2010 or August 5, 2010 from 2:00 p.m. to 7:00 p.m. (Remember to bring documentation such as tax returns, unemployment pay stubs, etc.)
Payment Plan
Cheryl McCarthy, District Payroll Clerk, will be happy to set up a payment plan (promissory note) for you. You may contact her anytime after May 10, 2010. District Offices are open all summer Monday through Thursday, 8:00 a.m. to 3:00 p.m. Her direct line is 708-583-5842.
No Computer?
If you do not have access to a computer, please visit the Elmwood Park Public Library, located at One Conti Parkway (708-453-7645). The library has computers for your use for a small fee. You may visit the High School Counseling Office over the summer to complete forms and pay fees in person. The High School Counseling Office is open all summer Monday through Thursday 8:00 a.m. to 3:00 p.m.
