NEW STUDENTS (Students who did NOT attend Elm Middle School or Elmwood Park High School last school year. Elmwood Park residents only.)

First day of school:
Tuesday, August 17, 2010: 7:50am - 3:00pm
Wednesday, August 18, 2010 and every Wednesday during the school year:
9:00 a.m. to 3:05 p.m.
Thursday, August 19, 2010 and every Monday, Tuesday, Thursday, Friday:
7:50 a.m. to 3:00 p.m.
RETURNING STUDENTS
(Students who attended Elm Middle School or Elmwood Park High School last year)
1. PRINT and fill out the forms listed below for EACH student:
- EPCUSD401 Proof of Residency Form Returning Students
- EPCUSD401 Student Data Form
- ISBE & U.S. Dept. of Education Race & Ethnicity Data Standards Memo & Form (Read the memo and complete the form.)
- EPHS 2010-2011 Payment Schedule*
2. MAIL your completed forms, proofs of residency and payment to:

OR, DROP OFF your completed forms, payment and proofs of residency in the High School Counseling Office anytime after May 10, 2010.

If you are applying for a fee waiver please come to open registration on August 4, 2010 or August 5, 2010 from 2:00 p.m. to 7:00 p.m. (Remember to bring documentation such as tax returns, unemployment pay stubs, etc.)
Payment Plan
Cheryl McCarthy, District Payroll Clerk, will be happy to set up a payment plan (promissory note) for you. You may contact her anytime after May 10, 2010. District Offices are open all summer Monday through Thursday, 8:00 a.m. to 3:00 p.m. Her direct line is 708-583-5842.
No Computer?
If you do not have access to a computer, please visit the Elmwood Park Public Library, located at One Conti Parkway (708-453-7645). The library has computers for your use for a small fee. You may visit the High School Counseling Office over the summer to complete forms and pay fees in person. The High School Counseling Office is open all summer Monday through Thursday 8:00 a.m. to 3:00 p.m.
