Returning students:

2011-2012 returning student registration forms were mailed home in March.  You have the option to pay your school fees with a credit/debit card online then mail COPIES of your proofs of residency to the address below, OR, you may mail a check/money order along with COPIES of your proofs of residency to:

 Registration is not complete until payment and residency are received.  Fees must be paid in full by August 31, 2011 to avoid collections and an additional 40% fee. Walk-in/drop off payment will not be accepted.

Open registration for returning students will be held on Wednesday, July 20, 2011 from 2 p.m. to 7 p.m. at Elmwood Park High School.  Waiver requests will not be honored on this date. (See below for more information on waiver requests.)  

 If you misplaced your forms, then:

 1.    Print and fill out the forms below. (Visit the Elmwood Park Public Library, 1 Conti Parkway, 708-453-7645, if you don’t have access to a computer or printer.  There is a small fee for printing.)

 2.    Mail copies of your proofs of residency along with payment to Kim Kaminski at the address listed above.  You have the option of paying online by credit/debit card—but you are required to mail in copies of your residency documents.  Registration is not complete until payment and residency proofs are received.  Fees must be paid in full by August 31, 2011 to avoid collections and an additional 40% fee. Walk-in/drop off payment WILL NOT BE ACCEPTED.

Waiver:

To qualify for a waiver, you must meet the state income eligibility requirements AND provide the following documents for ALL members in your household:

2010 completed tax return with schedules, W-2s, interest, etc.

Recent pay stubs if you receive unemployment, workman’s comp, etc.

Bank statements showing direct deposit of social security, disability, etc.

Any other documentation (bank statements, 401K withdrawal, etc.)
