Where to Send FOIA Requests

Pursuant to 5 ILCS 140/4b, portions of which require a public body to post on its website the name of its Freedom of Information officer and the address where requests for public records should be directed, District 401 provides the following directory information:

Although members of the public are not required to use a specific form when submitting a FOIA request, District 401 has prepared a form that may be submitted for this purpose:
FOIA Requests Are NOT NEEDED for Transcripts
There is no charge for current students to request their transcripts. For graduates or former students there is a $5 fee per request. Please allow three days for processing.
