Health Exams & Immunizations
District 401 students must present proof they have undergone all health examinations and immunizations as required by state law and District policy.
Failure to comply by the first day of school will result in the student's exclusion from school until the required health forms are presented to the District. (See Section 7:100 in the Board of Education's District Policy Manual.)
New students who enroll after the first day of school shall have 30 days following enrollment to comply with all applicable health examination and immunization regulations.
Please note that any homeless child shall be immediately admitted, even if the child or child’s parent/guardian is unable to produce immunization and health records normally required.

All students — new or returning — must fill out and submit this annually updated document:
All immunizations must be current. Any student who is new to District 401 must submit the following up-to-date documents (except preschoolers, who do not need to submit an eye exam report):
- Illinois Certificate of Child Health Examination Form
- Illinois Eye Examination Report (not required of preschoolers)
- Student Medical Information Form
In addition, if a new student is entering kindergarten, 2nd grade or 6th grade, a dental examination is required:
The State of Illinois requires all new students to present the Illinois Department of Public Health Certificate of Child Health Examination at the time of their entrance. It is expected that all students have the appropriate forms on file with the school health office. Please note that your child may not be able to enter the building for in-person learning if this information is not up-to-date and on file with the health office at your school. To review, all new students must satisfy the following requirements unless otherwise noted:
- A physical examination and immunization history dated within one year of school entrance
- A LEAD RISK assessment for children ages 6 and under
- The "Health History" section of the Child Health Examination Form completed and signed by the parent/guardian
- The "Physical Examination Requirements" section of the Child Health Examination Form completed as required, including diabetes and TB assessments and signature of health care provider
- The doctor’s stamp/signature, including phone and fax number
- An eye exam by an eye doctor (for kindergarten and all new students except preschool)
- District 401 Student Medical Form (annual form) completed and signed

- 2 varicella vaccines
- 2 MMR vaccines
- Tdap vaccine
- 2 doses of the meningococcal vaccine; if the student's first dose is received on or after their 16th birthday, only one dose is required
- District 401 Student Medical Form (annual form) completed and signed
If a Student is Homeless
As noted above, any homeless child shall be immediately admitted to District 401, even if the child or child’s parent/guardian is unable to produce immunization and health records normally required for enrollment.
