Pursuant to 5 ILCS 140/4b, portions of which require a public body to post on its website the name of its Freedom of Information officer and the address where requests for public records should be directed, District 401 provides the following directory information:

FOIA Requests Are NOT NEEDED for EPHS Transcripts
If you would like to request EPHS transcripts, please visit the Transcript Requests page and follow the steps as listed. You DO NOT have to submit a FOIA request.
For current students there is no charge for transcripts. For graduates or former students there is a $5 fee per request. Please allow three days for processing.
Please note that diplomas are not available for re-order. Graduates can only order transcripts.
