Parents and guardians, we invite you to help us make John Mills Elementary an even better school by completing the Illinois 2018-19 5Essentials Survey.

The results of this online survey will provide us with feedback on what areas our school leadership team is excelling at and where growth is still needed.

You can access the 5Essentials Parent Survey by clicking this link (UPDATE: This link has been disabled) and choosing "Parent Survey" in English or Spanish.

The survey will be given from Dec. 11, 2018, to Feb. 15, 2019.

The Illinois State Board of Education administers the survey in partnership with UChicago Impact. If you haven't had an opportunity to take the survey yet, please follow the directions provided below. We thank you for your time!

What Is the 5Essentials Survey?

The survey was developed by the University of Chicago Consortium on School Research and is derived from UChicago Impact's 5Essentials System for school improvement. 

The 5Essentials System is an evidence-based framework that identifies five factors that can lead to improved student outcomes. These five essential factors are:

► Effective Leaders
► Collaborative Teachers
► Involved Families
► Supportive Environment
► Ambitious Instruction

Researchers have found that schools strong in at least three of the five essentials were 10 times more likely to show substantial gains in student learning over time than schools weak in three or more of the five essentials.

How Can You Help?

Separate versions of the 5Essentials Survey are available for students, teachers and parents.

The parent survey is available in English and Spanish and takes approximately 10 minutes to complete. You may skip questions you do not wish to answer, but the survey must be completed in one sitting and cannot be retrieved once it is submitted.

If you decide to take the survey, your identity will be kept confidential and your responses will not be linked to you.

The parent survey requires a 20-percent response rate in order to generate a summary report.

If John Mills Elementary School successfully meets this 20-percent requirement, we will receive our report in March 2019. The parent survey results will also be available to the public as supplemental measures on the 5Essentials School Report.
