Positive Health Services in partnership with CCDPH is hosting a vaccination event at Elm Middle School.
District 401 students 12 and older will be able to get the Pfizer COVID-19 Vaccine at Elm on Monday, May 17, for shot 1 and Monday, June 7, for shot 2.
Students under 18 must be accompanied by a parent or guardian on both dates in order to receive the vaccination shots.

You can select the time that works best for you. Appointment times will be from 3:30 to 7:30 p.m.
If you do not have insurance, you can click "Insurance" and under the type of insurance type in "COVID HRSA Uninsured Testing and Treatment Fund."
Enter through Door 1 at Elm Middle School at your scheduled time.
